Policies and Forms
Whether it is taking part in the Physical Education program or one of the extra-curricular activities, there policies and expectations for all of those involved. Please take time to familiarize yourself with these policies. There will be multiple contracts that will have to be signed in order to participate.

Fair Play Basic Principles
Required BY ALL PARTICIPANTS
Ever since it's introduction on the international stage in 1963 by UNESCO, multiple governing bodies have adopted the principles and applied them to their specific sporting organization. All students and student-athletes must have this form signed to participate.

Student-Athlete Contract
Required BY ALL STUDENTS WHO WISH TO PARTICIPATE IN ANY TEAM
Any student who is interested in trying out or participating for any of the school teams should first download the following form and have it signed and ready for the coaches upon the first participation.
This contract outlines the expectations of student-athletes.
Physical Education Daily Assessment Form
This is the form that will be used by Physical Education teachers to assess performance of students daily. At the bottom you can see the codes and categories of what the teacher is looking for. How this is displayed in the class will be different depending on the unit and topic of conversation.
It is suggested that parents and students look at this form together to better understand the expectations of student conduct while in class.
It is recommended that you download the form and take a look.
Fitness Testing -Beep Test-
Assessment
3 times a year (Beginning, middle, end) we will be conducting fitness testing. Attached here is the form that will be used to assess the fitness level achieved by the students. Students will be notified in advance as to when this training will occur. It is suggested that students come prepared with appropriate exercise clothing, footwear and water bottle.
